Birth Certificates

You may request a birth certificate by phone at 229-995-5515, or by completing the printable form below.
If you submit your request by mail, YOU MUST include payment, a phoyocopy of ID, and a self-addressed stamped letter-sized envelope with your request.
Birth Certificates are $25 (+ $5.00 for each additional certificate requested at the same time.)

Death Certificates

You may request a death certificate by phone at 229-995-5515, or by completing the printable form below.
If you submit your request by mail, YOU MUST include payment, a phoyocopy of ID, and a self-addressed stamped letter-sized envelope with your request.
Death Certificates are $25 (+ $5.00 for each additional certificate requested at the same time.)

Gun Permits

To renew or apply for your weapons carry permit, click the link below to complete and submit the online application. YOU MUST follow up your application by making an appointment with the Probate Court to have your fingerprints and picture taken for your permit. APPOINTMENTS ARE MADE FOR MONDAY, TUESDAYS, AND WEDNESDAYS ONLY.
